What yeast is it ? I'd just take a refractometer reading, or drop in your hydrometer.

the hydrometer reading is still the same as the og was

Back in my beer kit days I had a Muntons dried yeast fail . Rehydrate for 15 minutes as usual before pitching .... absolutely zilch, just a sludge.

Yes, add another packet. I keep a pack of Gervin is the fridge in case my brews don't take off. Gervin is Nottingham Ale Yeast - get it from Wilko's for about £1.70.

I'd just add to vacant's comment, put the yeast in a little warm water to ensure that it is re-activated before you pitch it in the beer. Then you know its good yeast.

Don't Underpitching either. If your wort gravity is high then pitch 2 packs.

If things are not ideal then yeast can lag quite a bit, but three days with no change in OG is too much. Other competing things can gain a foothold given time, so you should get plenty of working yeast in there quickly.
Its really quite uncommon for dry yeast to do nothing at all. Unless the sachets are 'very' old and/or been kept in an oven, it could be worth reviewing your process as some kind of user error seems at least possible. e.g. pitching when the wort was still too hot would have had this result.
