A primary regulator is attached to the cylinder and capable of handling the 800psi of a full bottle.

A secondary regulator is attached to the low pressure side of a primary regulator, it cannot be attached to the cylinder or used at 800psi without blowing the safety valve.

You don't need a secondary regulator unless you want to run 2 kegs at different pressures.

I think also that secondaries tend to have better fine control of pressure. They may run from 0-120psi or so and allow you to (reliably) set the pressure with a finer "granularity" - i.e. 12, 13 or 14 psi, whereas the primaries may have less precise control and you'll be lucky to reliably get it set at 10, 20 or 30...

Depends on the reg though....
