Counter flow chillers work by having the hot wort flow through a metal tube which is inside another tube through which cold water flows.

The wort flows in one direction while the water flows in the other. You can make a cheap one with a length of copper microbore fed down a garden hose (which I believe is Jim's weapon of choice).

If you get the flow rates right and have a long enough flow you can get your wort coming out of the chiller at pitching temperature.
Ingredients: Water, Barley, Hops, Yeast, Seaweed, Blood, Sweat, The swim bladder of a sturgeon, My enemies tears, Scenes of mild peril, An otter's handbag and Riboflavin.
