Re-hydration in a third of a cup of cooled boiled water is the best method. Then you just pour it in the wort and there's no need to stir. If the yeast doesn't foam up when it's in the cup after 20 minutes then you know you have a dud one.

I sprinkle, then stir lightly, as the wort is quite frothy, and seeing as the spoon is there already, I stir it. Sprinkling alone is fine too. I read somewhere, "the yeast will find it's way",but I prefer to help it by stirring, otherwise it sits on the froth.

I have always just sprinkled my yeast on top of the foam it rehydrates with the moisture from the foam and also it's o2 requirement for the first stage of fermentation.

Maybe I trust the yeast producers to much but touch wood never had a problem with dead yeast yet.
